Stratus Forms - Form Builder
STRATUS FORMS - FORM BUILDER IS IN PREVIEW! PLEASE USE CHROME BROWSER WHEN USING THIS FORM AT THIS TIME!
Before creating a Stratus Forms form make sure you have done the following:
1. Create a SharePoint list to store the form data.
2. Create a Multi-Line PLAIN TEXT field in the list with the name "StratusFormsData"
3. Identify a document library to store your form scripts in (I use "Site Assets")
STEP 1: Create the fields for your Form
Single Line of Text
Multiple Lines of Text
Drop Down List (Hard coded choices)
Drop Down List (Choices from SharePoint List)
Social Security Number
Repeating Content Container
Radio Group Name:
Source List to Pouplate Drop Down List:
Field from Source List to use in Drop Down List (use INTERNAL Field Name):
Text to use for first "unselected" entry:
If you wish to promote this field to a corresponding SharePoint List field, enter the INTERNAL NAME of your SharePoint Form Field here:
STEP 2: Enter the name your SharePoint List and click the "Geneate Script Source" button
STEP 3: Take the generated HTML from the "Form Source" tab to your designer to make it "pretty" (be careful not to change the generated attributes) and store the HTML in a file in your document library.
STEP 4: Store the script in the "Script Source" tab in a document library.
STEP 5: Create a web part page in SharePoint. Add a Content Editor Web part and link it to the the form html file from step 3. Add another Content Editor Web Part to the page and link it to the Script Source file from Step 4.
